Heart Assess Program
Holy Redeemer’s Heart Assess is a program that helps patients with heart disease better manage their symptoms and medication for improved quality of life. Home health care nurses are an integral part of the program, helping patients with medication and diet management; monitoring critical health readings and assisting with lifestyle changes.
Remote Monitoring
For patients with chronic conditions that require daily monitoring such as heart failure, chronic obstructive pulmonary disease (COPD) and diabetes, remote patient monitoring can improve outcomes and quality of life. Used as a tool to effectively manage the disease, remote monitoring allows healthcare professionals to treat the condition before it worsens, reducing the number of emergency room visits and hospital readmissions.
