 Holy Redeemer's LifeAssess program is the first of its kind in the nation for older adults age 85+, to be certified by the Joint Commission on the Accreditation of Health Care Organizations (JCAHO). With older adults living longer than ever before, Holy Redeemer's compassionate and experienced home care teams recognized the need for a program that would identify and treat four major complex geriatric syndromes:
- Depression
- Dementia
- Falls Risk
- Urinary Incontinence
For adults age 85+, these common syndromes may threaten the ability to live safely at home and lead to premature placement in a nursing or long-term care facility. With the LifeAssess program, Holy Redeemer's goal is to identify and treat those at risk, early in the care process, prolonging the ability to remain independent at home, and forestalling the need for skilled care placement.
Could you or a loved one benefit from LifeAssess? Ask yourself or your loved one these questions:
- Have you had decreased interest in doing things over the past month?
- Are you unable to find pleasure in activities you used to enjoy?
- Does it take longer for than 30 seconds for you to walk 20 feet?
- Do you dribble or leak urine?
- Are your clothes or bedding ever wet?
- Do you smell urine, see stains or wear protective pads?
A positive response to any of these questions indicates that you or a loved one may benefit from a referral to LifeAssess.
Individuals who are already patients of Holy Redeemer's home care program, and are age 85 or older, automatically receive a comprehensive screening and evaluation, complete with recommendations and action plan.
