Our professional nurses and therapists offer a full range of home care services in southeastern Pennsylvania and 11 counties in New Jersey, including a wide range of therapies, hospice and palliative care, and unique programs for seniors like Falls Risk Assessment and LifeAssess, the first home care program for the frail elderly to be certified by the Joint Commission on Accreditation of Healthcare Organizations.
Services may include:
- Skilled nursing services, including IV and infusion therapy, wound care, ostomy care, incontinence program, cardiac rehabilitation, diabetes management, pre-hospice care, hospice care, and mental health.
- LifeAssess frail elderly home health assessment program
- Certified home health aides
- Physical, occupational and speech therapy
- Pastoral care/medical social services
- Inpatient Hospice Services*, including respite care and bereavement support
- 24 hours-a-day, seven day-a-week availability
In addition to our highly qualified and experienced nursing staff, Holy Redeemer Home Care & Hospice's home health aides are extensively screened and receive in-depth training and education to meet all Medicare requirements. Plus, our non-profit agencies are Medicare- and Medicaid-certified and accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO).
Personal Care and Housekeeping Assistance*
Our home health aides and companions provide a helping hand with bathing, dressing, grooming and meals as well as household chores: patient laundry, light housekeeping and local errands. Each personal care client receives a thorough assessment from a registered nurse who develops a comprehensive care plan.
